Coaching Agreement for The Reset & School of Momming ADHD

This Coaching Agreement is entered into between Focus Forward Coaching, LLC (“Coach”) and the participant (“Client”) enrolling in The Reset coaching program.

By completing registration and submitting payment, Client acknowledges and agrees to the terms outlined below:

1. Program Description

The Reset is a six-week coaching program that includes:

• Daily accountability emails with ADHD insights, reflection prompts, and connection challenges

• Weekly group coaching sessions held virtually

• Twelve (12) months of continued access to the School of Momming ADHD program, beginning from the date of purchase, which includes community support, additional resources, and ongoing group coaching opportunities

2. Payment Terms

The total program fee is $3000, payable in full at the time of enrollment. Payment is non-refundable except as stated in this agreement.

Payment can be made via credit card. All payment information is securely handled.

Failure to submit payment may result in removal from the program and access being suspended.

3. Session Attendance and Cancellations

Group coaching calls are scheduled and shared with participants in advance. While attendance is encouraged for full benefit, all calls are recorded and made available for replay.

There are no makeups or refunds for missed calls. Cancellations do not result in partial refunds or credit toward future services.

4. Confidentiality and Privacy

Coach agrees to keep all information shared by the Client in the coaching relationship confidential, with the following exceptions:

• If disclosure is required by law

• If the Coach believes there is risk of harm to the Client or others

• If information is subject to subpoena

No Client information will be shared with third parties (such as schools, therapists, or physicians) without written permission.

5. Permission to Record

Group coaching sessions may be recorded for the purpose of sharing replays with other participants and for internal training or supervision. By participating, Client consents to being recorded and understands recordings may be viewed by other members of the program or coaching team. All recordings are stored securely.

6. Nature of Coaching and Disclaimer

Coaching is a partnership focused on personal and parenting development, growth, and clarity. Coaching is not therapy, counseling, or mental health treatment, and does not diagnose or treat medical or psychological conditions.

Client agrees to participate fully and understands that results may vary based on their level of engagement and individual circumstances. No guarantees are made regarding outcomes.

7. Emergencies

The Coach is not a crisis provider. In case of a physical or mental health emergency, the Client should contact emergency services or their healthcare provider directly. Do not rely on communication with the Coach for urgent or life-threatening issues.

8. Disputes

If a dispute arises between the parties, both agree to attempt resolution in good faith. If unresolved, and legal action is taken, the prevailing party may recover reasonable legal fees and court costs.

This agreement is governed by the laws of the state of Kentucky.

9. Termination and Access

Client may withdraw from the program at any time; however, no refunds will be given for withdrawal after program access has begun.

Coach reserves the right to terminate access if Client behavior is disruptive, harmful to other participants, or in violation of the agreement.

10. Entire Agreement

This document represents the entire agreement between the Client and Focus Forward Coaching, LLC with respect to The Reset and School of Momming ADHD. No verbal agreements or prior understandings shall be valid unless made in writing and agreed upon by both parties.
